The Transition Service helps individuals who qualify to transition into adult social care. The team conducts a Needs Assessment to determine the appropriate adult care package. This plan is implemented before the individual turns 18, ensuring all necessary support is ready for them upon adulthood.
There is a lack of a clear pathway for young people stuck in limbo from the ages of 14 to 18. They are trapped in safety plans and assessment outcomes that they have outgrown and cannot receive a new care package before turning 18.

A smooth shift to adult mental health services involves collaboration between Child and Adolescent Mental Health Services (CAMHS) and adult services, with family involvement being vital to ensuring continued support.

As individuals transition into adulthood, assistance may be needed to secure employment and suitable living arrangements. We work with Disability Employment Advisors and local authorities to provide tailored guidance and support housing options that may be available for those requiring care while fostering independence.
