Social Care Review

It is a legal obligation for a Local Authority to provide an annual review of an individual’s Care and Support Plan (or sooner if an individual’s needs have changed). This Social Care Review will establish if the previous support plan outcomes are still effective and relevant.

Our Independent Social Workers will provide a review to ensure the care and support is completely appropriate for the individual’s current needs.

This Review will detail:

  • The progress of goals and achievements detailed
    in the previous plan
  • Any better ways to support, ie – services,
    equipment etc.
  • If and how support needs have changed and
    whether this means a reassessment is required

 

These reviews will be undertaken in the most appropriate location by an Independent Social Worker with the most relevant training and experience.

Once a referral has been received, we will provide the CV’s, timescales, and estimated costs of the most suitable Social Worker for your specific case.

We can provide an independent review of an individuals Care and Support Plan, as instructed by a Local Authority as part of an annual review or as an impartial assessment instructed by a Solicitor.
