Continuing Healthcare

All our Independent Social Workers’ complete assessments and reports in line with the National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care. 

Continuing Healthcare Checklists and Full Assessments are completed where they have been requested by an Individual through their Solicitor or by the Local Authority or where the Independent Social Worker has identified a potential need during the completion of another assessment.

The purpose of the process is to assess eligibility for NHS funded Continuing Healthcare.

Stage 1 (Continuing Healthcare Checklist)

The Continuing Healthcare Checklist is the first step in the process of assessing potential eligibility for NHS Continuing Healthcare.  

This step is used to decide whether the ISW believes based on the evidence available to them that the individual has significant Healthcare Needs in 11 Domain Areas.

The ISW completes a summary for each section of the Checklist that is relevant to the individuals Healthcare Condition and goes on to balance the identified ratings for each section to conclude whether a Full Assessment (Decision Support Tool) is required.

All Checklists are completed in a person centred way involving the individual and their carer, representative or advocate. 

Stage 2 (Decision Support Tool, Full Assessment)

If the completed Checklist identifies that a Full Assessment is needed the Independent Social Worker will work with members of the MDT, the individual, their representative or advocate to arrange a meeting to review all the evidence.

During the Full Assessment meeting the Independent Social Worker will be fully involved in the discussion, decision making and completion of the Decision Support Tool.

The Full Assessment will consider the nature, intensity, complexity and unpredictability of the individuals healthcare needs. The tool is a record of the evidence gathered and used to make recommendations and determine eligibility for NHS Funded Continuing Healthcare. The Full Assessment and Recommendations are reviewed by the Integrated Care Board.
