Care and Support Plans

Care and Support Plans are carried out following the completion of a Needs Assessment or Review where a right to eligible services is determined.

Through our nationwide panel of Independent Social Workers, we can support you and your team by providing a robust formulation of the eligibility for care and support needs.

Our Care and Support Plans include:

  • All needs identified by a Assessment
  • Which needs are eligible using the National Eligibility Criteria
  • Which needs the Local Authority is going to meet and how it’s going to meet them
  • The Personal Budget and how this would be managed
  • Advice and Information about what can be done to meet or reduce the needs in question
  • Advice and information about what can be done to prevent or delay the development of needs for care and support in the future
  • The outcomes the person is likely to achieve because of the Care and Support Plan

Our panel of Independent Social workers cover a wide range of disciplines including:

  • Learning Disabilities
  • Autism
  • Mental Health
  • Older Adults
  • Physical Disabilities

 

Once a referral has been received, we will provide the CV’s, timescales, and estimated costs of the most suitable Social Worker for your specific case.

At Advanced ACA we are able to support Local Authorities and Solicitors by providing a range of assessments, such as Needs Assessments, Carer’s Assessments and Social Care Reviews.

This allows us to provide a consistent presence for the individual, should they then require a Care and Support Plan thereafter.
