Our panel consists of highly qualified Independent Social Workers with a range of specialisms, including learning difficulties, autism, mental health, older adults and physical disabilities. This ensures that we can present the most appropriate Social Worker to meet the requirements of your case and suit the needs of the individual.

Our assessments are provided in a timely manner to ensure prompt decisions can be made regarding the best outcome for the individual.

Needs Assessments

Our Independent Social Workers will collate a full report to determine the individual’s eligibility for care and support. We ensure the assessment is adapted to suit the unique requirements of the adult.

Care and Support Plan

Care and Support Plans will include all needs identified, eligibility, Personal Budgets and Information and Advice.

Social Care Review

We can provide a review to determine if the previous support plan outcomes are effective and relevant. Our review will ensure the care an individual receives is pertinent to their needs.

DoLS

A Deprivation of Liberty Safeguards (DoLS) assessment evaluates whether an individual qualifies for legal protections related to restrictions on freedom. Conducted under the Mental Capacity Act 2005, these assessments ensure that any deprivation of liberty is in the person’s best interest and necessary. 

Psychology Assessments

Psychological evaluations assess an adult’s mental health and behaviour, providing insight into daily functioning. In adult social care, these assessments help identify specific needs and determine if additional support services are necessary to ensure their well-being and independence.

Transitions Services

Transition services assist young individuals moving from children’s to adult social care. These services address a broad spectrum of needs, including health and social care, mental health, education, financial benefits, employment, and housing, helping ensure a smoother transition and continued support during this critical life stage.

Carers Assessment

This assessment will identify the Carer’s Needs and will determine any impact the caring role may have on their health and wellbeing.

Continuing Healthcare

All our Independent Social Workers’ complete assessments and reports in line with the National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care.
